Palm Beach, USA: A 53 year old woman, who has battled depression and anxiety for years, was shocked to find, on a written summary attached to her medication, that she was "CrAzY!!", it is reported in the South Florida Sun-Sentinel.

In another field the following text appeared:

"She's really a psycho!!! Do not say her name too loud, never mention her meds by names & try to talk to her when ... "

The printout then goes over to another sheet which has not been released.

The notes field of the prescription is normally just a note for the pharmacist and isn't intended to be shown to the customer, a spokesperson for the chain told the newspaper.

The woman, who we will not name, is suing the supermarket (a national chain in the USA) for defamation, negligent supervision and intentional infliction of emotional distress.
